Description: Professional accountant responsible for complex financial data, accounting systems, methods, reports, and the evaluation and interpretation of financial data in accordance with Generally Accepted Accounting Principles (GAAP) and Governmental Accounting Standards Board (GASB) standards. Responsible for the integrity of the general ledger. Examines, analyzes and interprets accounting records for management. Maintain accurate accounting data and accurate reconciliations of accounts and fund balances involving the general and operating ledgers. Description: Works within Project Choices, a managed care system designed to serve the elderly and the disabled who are at-risk of entering a nursing home by exploring their options and alternatives to institutional care. Care Managers complete comprehensive assessments of participants, with a strong emphasis on Person Centered Planning and client direction. Many of the specific duties listed below are mandated by the Michigan Office of Services to the Aging (OSA) Care Management Performance Objectives Essential job functions: (Reasonable accommodations will be provided, if necessary, for individuals with disabilities who can perform the essential job functions.) Assists clients with securing Medicaid eligibility for Waiver services. Using the principles of Person Centered Planning, develops care plans for each client addressing the individual’s specific needs and desires. Conducts assessments as a member of a professional team. As Primary Care Manager, monitors the cost of Medicaid Waiver services and other formal services to maximize resources and stay within program parameters and goals. Maintains accurate files with all the mandatory documents including OSA and Medicaid Waiver documentation. Provides advocacy on behalf of clients to secure services and benefits to which they are entitled and promotes client freedom of choice. Updates knowledge in gerontology and chronic illness management through attendance at agency meetings and continuing education opportunities. Covers 24 hour on-call services as scheduled.
