Case Manager

Description: Blue Care Network of Michigan is seeking an Advantage Nurse Care Coordinator (Case Manager) who utilizing a collaborative process, will assess, plan, implement, coordinate, monitor, evaluate and advocate the individual’s health care options and services through available resources to promote quality, cost-effective outcomes. The case manager shall assist in identifying appropriate providers and facilities across the continuum of services while ensuring that available resources are being used in a timely and cost effective manner to obtain optimum value of both the member and the reimbursement source. The BCN Advantage Nurse Care Coordinator (Case Manager) will focus on BCN Advantage members during and after hospitalization to assure that the discharge to home is safe and successful. The nurse care coordinator will make on site visits with members during their hospitalization to help facilitate a safe transition to home. Following discharge home; the nurse care coordinator will contact the members post discharge, to encourage follow up with their physician, assess and educate on the members understanding of medications and discharge instructions and to assist with providing health care services and available resources to the member in an effort to prevent unnecessary readmissions to the hospital. - Evaluate proactive triggers to identify potential members suitable for effective case management/disease case management intervention. Identify practice patterns and trends wherein member outcome can be positively influenced. Determine what patterns of care may lead a member into a case management program. Analyze patterns of care that may be associated with progression to severe disease or declining condition. Identify and focus upon active cases with high financial impact in order to monitor and manage continuum of care. - Conduct a thorough and objective evaluation of the member’s current status. - Gather all relevant data and obtain information by interviewing member/family, contacting appropriate medical and supportive personnel, research benefits and performing careful evaluation of the entire situation. Responsible for administering, tracking, follow up, data collection and reporting associated with health risk appraisal process. Develop a safe treatment plan, which enhances member outcome and reduces BCN liability. Assess resource utilization and cost; diagnosis, past and present treatment course and services; prognosis, goals, treatment, and provider options. Assist facilitation of services and coordination of care when necessary. Recommend referral sources and provide member/family education through condition/disease specific literature and resources. Establish measurable case management goals which promote evaluation of the cost and quality outcomes of the care provided. Represent the member’s best interests. - Identify and coordinate changes in practice patterns and treatment plans to bring about appropriate care and cost effective outcomes. - Coordinate, monitor and document the quality of care and/or the quality of life improvements as evident by the services and products delivered to the member, to determine if the goals of the care plan are being achieved, or if those goals remain appropriate and realistic. Compare the member’s disease course to an established critical pathway to determine variances. - Actively promote communication between member, family, providers and all parties involved. - Identify and documents QA issue and resolves or routes to QA for handling. - In conjunction with Provider Services, identify and negotiate contracted rates with non-contracted providers for services required - Promote and engage in positive and constructive daily team work through consistent interaction with team members and perform other duties as required. Requirements: - Bachelor’s degree in nursing or health related field or case management certification preferred. - Registered Nurse with current Michigan license. - Must maintain continuing education appropriate to case management and professional licensure. - If not certified, application for certification through the Commission for Case Manager Certification (CCM) required within one year of hire with successful completion within two years of hire. - Two (2) years of acute patient care experience with a broad clinical background required. - Two (2) years of case management experience required. Will consider discharge planning, utilization management, quality assurance, home health care or community/public health experience. - Experience in a managed care environment preferred. - Candidate must possess knowledge of the following: - Medical management processes across the continuum of care required.
- Current medical procedures/practices and their application, including alternative care settings and levels of services well as current trends and developments in medicine and nursing. - Disease processes and recognition of their distinctive cost patterns and unique range of available therapies and interventions. - Cost containment strategies regardless of limitations of policy/benefits available to the member. - Local/national health, social services, funding and community resources preferred. - Medical terminology and ICD-9 and CPT coding required. - Managed care philosophy, policies, and procedures preferred. - BCN/BCBSM policies, procedures, and benefits preferred. - State and Federal HMO regulations preferred. - HIPPA privacy standards required. - Must be able to identify cases appropriate for case management, assess the member, develop a plan in cooperation with the primary care physician and other providers. - Strong problem-solving, decision making, organizational, planning and implementation skills. - Excellent customer servicing and positive interpersonal/relationship building skills required. - Effective written and verbal communication skills required. - Well developed time management and prioritization skills required. - Creative and resourceful with good negotiating skills required. - Must be able to work with minimal supervision. - PC literate - Registered Nurse with current Michigan license. - Bachelor’s degree in nursing or health related field and case management certification (CMC) preferred. If not CMC certified application for certification through the Commission for Case Manager Certification (CCM) is required within one (1) year of hire. - Two (2) years of case management experience is required, but will consider discharge planning, utilization management, home health care or community/public health experience. - Knowledgeable of Medicare Advantage and Part D programs desirable. - Ability to conduct telephone assessment of member and develop a plan of care. - Ability to recognize categories of medications and ability to educate members regarding medication usage. - Knowledgeable in disease processes and able to recognize their distinct cost patterns and available therapies and interventions.
